At the September 1st meeting....

We viewed a FAA Safety DVD regarding the human factors involved in aviation safety.  What was specifically identified?  

Knowledge:  as pilots we need to be continually learning, listening to others, and learning from our mistakes and near-misses.  We can do this by reading journals, attending lectures, and simply talking with one another.

Communication:  utilizing our technology, verbalizing and demonstrating our intent.   

Fatigue:  physical stress, tiredness, dehydration, hunger...  All are our enemies.  It’s the “Go - No Go” decision.

Task Saturation:  repetitive procedures or intense complexity can lead to mental fatigue

Complacency:  decreasing our vigilance to detail, checklists, and procedures.  It is easy to become distracted and lose track of time and traffic.  “Normalization of Deviance” which means that I didn’t do it and nothing went wrong, so I don’t do it again and again... Hoping that no error happens.

 

Errors are a chain of events.... Elimination of any link in the chain will prevent the error

 

Suggestions:  subscribe to a new periodical, attend seminars, train with an instructor, be involved with other aviators
